Northwest Family Physicians and Crystal Doctors Park have served their communities for over thirty years. In October of 1984, the two clinics merged and became known as Northwest Family Physicians. As family physicians, we offer care to all ages and have a network of specialists for referral when necessary. As you will note, we have three offices. Each of these is listed with their hours and phone numbers.
We are an independent clinic which prides itself on high quality providers and medical care. We continue to work on improving service to our patients. Northwest Family Physicians was recently awarded the Health Partners Outcome Recognition Award for excellent medical care in preventive services, diabetes and coronary artery disease. We continue to add services to benefit our patients. Our primary hospital is North Memorial Medical Center.
